PCOS Is Not Just a Period Problem — It Starts in the Gut

Most conversations about PCOS center on hormones. Elevated androgens. Irregular cycles. Insulin resistance. These are real and they matter. But if you have been working on your hormones for years without lasting results, there is a question worth asking: what is happening upstream? The gut is where hormones are metabolized, recycled, and cleared. A specialized collection of gut bacteria called the estrobolome produces an enzyme — beta-glucuronidase — that determines how much estrogen gets reabsorbed versus excreted. When the gut microbiome is disrupted, this process goes sideways. Estrogen that should be cleared gets recirculated. The hormonal picture becomes more complicated, not less. For women with PCOS, this matters enormously. Many PCOS presentations involve estrogen dominance alongside androgen excess — a combination that is nearly impossible to address through hormone support alone if the gut is not functioning correctly. You can take all the DIM and progesterone you want. If your gut is not clearing estrogen efficiently, the underlying imbalance persists. There is also the gut-insulin connection. Gut dysbiosis drives systemic inflammation, and inflammation drives insulin resistance. Insulin resistance is one of the primary drivers of androgen excess in PCOS. This is not a coincidence. These systems are connected. Functional nutrition for PCOS does not start with the hormones. It starts with a full picture of what is happening in the gut, the adrenals, and the metabolic system — using tools like the BiomeFX gut microbiome test, the DUTCH hormone panel, and the OAT to assess mitochondrial function and neurotransmitter production. When you address the root, the hormones follow. That is the difference between managing PCOS and actually changing its trajectory. If you are tired of protocols that only address part of the picture, [book a free strategy call](https://jackiehackett.com/vsl) to talk through what a comprehensive investigation would look like for you.
